Canada IMG Pathways
3 ways to practice medicine in Canada
Every pathway starts with clinical skills. Understand your options — then start simulating the encounters that matter for your specific route.

|---|---|---|---|
| Timeline | 4–7 years from MBBS | 12–24 months from application | 12–18 months from application |
| Required exams | MCCQE Part I → NAC OSCE → CaRMS Match | MCCQE Part I + NAC OSCE + language tests → PRA assessment | MCCQE Part I required (AB) + provincial competency assessment |
| Provinces available | All provinces | AB, BC, SK, MB, NS, NB | AB (established), BC & ON (expanding) |
| Typical income | Resident salary ($60–80K CAD/yr during training) | $180–250K CAD/yr (rural/underserved areas) | $100–160K CAD/yr (supervised role) |
The full Canadian residency pathway
The traditional route to full independent medical licensure in Canada. Highly competitive for IMGs — but the simulation skills and MCCQE Part I preparation you build here directly improve your CaRMS application.
Steps
- 1Pass MCCQE Part I (clinical decision-making exam)
- 2Pass NAC OSCE (clinical communication exam)
- 3Apply through CaRMS (Canadian Resident Matching Service)
- 4Complete 2–5 year residency in your specialty
- 5Obtain MCCQE Part II → full licensure
🌍 For international IMGs: Most international IMGs target this pathway. Your MBBS qualifies you to apply — strong MCCQE + NAC OSCE scores are your competitive edge.
Faster licensing in underserved communities
A province-administered supervised assessment that can lead to full licensure in underserved communities — without completing a full residency. Clinical communication and documentation are the core assessment criteria.
Steps
- 1Pass MCCQE Part I
- 2Pass language proficiency (IELTS/OET)
- 3Apply to provincial PRA program
- 4Complete 12-week supervised assessment in a rural/underserved setting
- 5Receive provisional license → practice in eligible communities
🌍 For international IMGs: Strong demand in Alberta, Saskatchewan, and Manitoba. IMGs already in Canada often pursue this as the fastest route to independent practice.
Start practicing medicine in Canada sooner
Associate Physicians work under the supervision of a licensed physician — seeing patients, writing notes, ordering investigations, and performing procedures. Canada's physician shortage is driving rapid expansion of this role.
Steps
- 1Pass MCCQE Part I (required in Alberta)
- 2Verify MBBS credentials through provincial authority (CPSA in AB)
- 3Pass provincial competency assessment
- 4Secure a supervising physician / health authority position
- 5Receive AP registration → begin supervised practice
🌍 For international IMGs: This is not a fallback — it's a different plan. Many IMGs practice as Associate Physicians within 12–18 months of arriving in Canada, while pursuing CaRMS in parallel.
