CPSO Registration: The Last Step That Takes the Longest
After passing MCCQE Part 1, NAC OSCE, and matching CaRMS, many IMGs assume CPSO registration is a formality. It is not. The College of Physicians and Surgeons of Ontario processes applications carefully — and the timeline can catch IMGs off guard if they do not plan ahead.
The Real CPSO Processing Timeline
The CPSO website states that complete applications are processed within 4–6 weeks. The operative word is complete. Most IMG applications are not complete on first submission — which is why the real-world timeline for many IMGs is 3–6 months from initial application to receiving a certificate.
What Causes Delays
Missing or Incorrect Documents CPSO requires certified translations of all non-English documents, original degree certificates (not copies), and current certificates of standing from every jurisdiction you have held a license. Missing even one document restarts the review clock.
Source Verification Gaps If your MCC credentials are not fully verified and in good standing, CPSO will not process your application. Verify MCC status before applying.
Outstanding Good Standing Certificates If you held a license anywhere else — another province, another country — you need a certificate of good standing from that jurisdiction. Some international medical councils take 4–12 weeks to issue these. Apply for them immediately.
Prior Licensing Issues Any prior licensing restriction, disciplinary action, or malpractice history — even if resolved — requires additional review. Disclose everything upfront. Non-disclosure discovered later causes far greater delays.
The Steps in Order
1. Complete your CaRMS residency match 2. Immediately apply for all certificates of good standing from prior jurisdictions 3. Gather all original documents and certified translations 4. Confirm MCC credentials are current and in good standing 5. Submit CPSO application with all supporting documents 6. Respond to any CPSO requests for additional information within 5 business days (delays in response restart wait times)
One Thing Most IMGs Miss
You can apply for a Postgraduate Education Certificate from CPSO to begin your residency while your independent practice application is being processed. This is a different certificate than the Independent Practice certificate — make sure your residency program coordinator applies for this well in advance of your start date.
Does This Apply Only to Ontario?
No — every provincial college has its own registration process. CPSBC (BC), CPSA (Alberta), and CPSM (Manitoba) have similar multi-step processes. Ontario tends to have the longest processing times due to application volume.
